The primary care provider will function as a highly skilled, independent care provider in the management of patients, you’ll be part of an interdisciplinary care team with access to state-of-the-art medicine and some of the most advanced medical technologies available. The ideal candidate will be an enthusiastic member of a multidisciplinary team in the Primary Care setting. Independent workstyle. Prior experience in primary care, internal medicine, family medicine or women’s health. The physician serves as a Primary Care Provider (PCP) at a Community Based Outpatient Clinic (CBOC). The CBOC operates under a Patient Aligned Care Team (PACT) Model.
