During months that service providers do not have the target amount of service planned the performance requirement will be 80% of planned services delivered.The therapist will ensure that their services will begin within 28 days of the initial IFSP 100% of the time. Document all efforts to overcome barriers to services beginning on time.Documentation This function requires that the therapist clearly and legibly document services provided to include ongoing progress notes, consultation notes, professional reports, correspondence, and IFSPs. All chart documentation will be turned in on a weekly basis by the established deadline 100% of the time. Professional reports will be turned in no later than the 5th working day of the month following the month the evaluation was completed.Write the majority of progress notes in the home with input from the family. All notes must contain required elements.MAC the therapist will participate in the Random Moment Time Study and the associated training as required.Professional Development - This function requires the therapist to successfully complete the professional training to maintain competency and meet the continuing education required by professional standards and the Center. Occasionally the staff person may need to drive within or outside the five (5) counties in order to participate in a special activity, such as a conference.Supervise and train students, interns, and other therapists as appropriate. This function requires the therapist to supervise and train students, interns, and other staff and therapists in therapeutic techniques for the pediatric population as needed by the program.Feedback from 95% of families served is positive. The feedback may be from parent satisfaction questionnaires sent out by the program, phone calls or other contacts made by supervisors to families, and/or from family-initiated contacts with the program or the state office.Support program events such as Child Find activities and other special events.Accept assignment of services to be provided in a language other than English as determined by the primary language of the referred/enrolled children and their routine caregivers as the needs arise.
